Assumption of Risk

In consideration of participating in fencing, I represent that I understand the nature of this activity and that I or any minors I enroll

are qualified, in good health, and in proper physical condition to participate in this activity. I acknowledge that if I believe conditions

are unsafe, I will immediately discontinue participation in the activity. I fully understand that this activity involves risks of bodily

injury which may be caused by my own actions or inaction, those of others participating in the event, the conditions in which the

event takes place, or the negligence of the releases named below I also understand that there may be other risks either not known

to me or not readily foreseeable and this time, and I fully accept and assume all such risks and all responsibility for losses, cost, and

damages I incur as a result of my participation in the activity.

Assumption of Liability

As the legal parent or guardian, I release and hold harmless Orion Fencing, its owners and operators and board members from any

and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including

death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the

control and supervision of Orion Fencing, its owners and operators or en route to or from any of said premises.

Payment Policy

Orion Fencing accepts payments of cash, check, credit card, and debit card. Selection of the Auto Pay price option requires that a

card be placed on file with Orion Fencing with the understanding that the card will be automatically charged on the first of each

month unless other arrangements are made at least five days in advance of this date. Declined cards, returned checks, and late

payments are all subject to fees: $35 for returned checks and declined cards, and a $10 late fee for payments made or authorized

after the first of the month. If payment is not made or arranged by the fifth of the month, the student will be dropped from the

class. Students attending classes without payment will be billed the prorated class fee amount for class(es) attended. Orion Fencing

homeschool classes are billed by the semester. The fees may be paid in full at the beginning of the semester or broken into five

equal installments due at the beginning of the semester and the first of the next four months of the semester. Homeschool class

fees are nonrefundable.

Medical Emergencies

I, the parent or legal guardian of the indicated minor(s), fully understand that Orion Fencing staff are not physicians or medical

practitioners of any kind. With the foregoing in mind and in the event that any kind of injury or illness should occur to myself or child

while on Orion Fencing premises, I hereby authorize Orion Fencing to render first aid as deemed necessary in their discretion and/or

to seek medical assistance, including calling 911 or otherwise arranging for the transport of myself or my child to an appropriate

facility for treatment. Additionally, I hereby authorize any trained and licensed medical professional to administer emergency

medical treatment to myself or my child should injury or illness occur in my absence or lack of lucidity. I understand that Orion

Fencing will make every effort to promptly notify me or my emergency contact in the event of any such emergency.

Photo & Video Release

I hereby authorize Orion Fencing to use photographs or video taken of myself or my child while participating in Orion Fencing classes

and/or activities in their marketing and training materials as well as on social media and websites. I understand that if my child or I

participate in a tournament that the performance may be videotaped and could possibly be broadcast on local television.
